There are two ways to enter the Five Below Survey Sweepstakes. When planning to enter via online submission, participants need to secure a receipt. One needs to enter the code to join the sweepstakes. The survey is currently hosted at FiveBelowSurvey.com. Here are the other requirements for both online and mail-in entries. It’s noteworthy that additional purchases will not give more chances of winning. The odds will still depend on the number of players that will join the sweepstakes.

To join the official Five Below customer satisfaction survey, you need to be a legal resident of states that has Five Below brick-and-mortar stores operating in. Any entries coming outside of the country are null and void. To participate, the entrant should be at least 18 years old at the time of submission. Employees, administrators, representatives, and their immediate family members are not allowed to join the sweepstakes, but they can leave their feedbacks and suggestions for the improvement of the store. All entries must be received by Five Below before the drawing date. Winners cannot convert the gift card to cash or other products. The spending of the gift card is only applicable to selected items.

- Answer the questions as honestly as possible.
- After the survey, opt to join the sweepstakes.
For those who don’t have a printed receipt but want to join the sweepstakes, they can submit your entries via mail. Take note that the entry should be postmarked properly and sent before the end of the entry period. Any delay, destruction, or possible loss of entry during transit is not the responsibility of Five Below. Proof of sending is not proof of receipt by the Five Below representative.
